Available immediately in English, the Incident Report form (Product No. 10120 Rev. 01/20 Ver 6.0) has been updated to allow for easier completion and review. It’s a fillable PDF that can be easily emailed to your PADI Regional Headquarters to quickly report the incident. The new Incident Report Form is available on the PADI Pros’ Site.
Remember that if the incident occurred during training, submit copies of all student diver paperwork along with the report form. For nontraining incidents, include copies of releases, equipment rental forms, boat rosters, etc.
